PAYMENT TYPES

The following are the checklist of documents required for processing all benefit types accessible on the Retirement Savings Accounts of employees

Customers with RSA balance above N550,000; Lump sum Payment (optional) and Programmed Withdrawal/Annuity:
  • a. Official notice of retirement from employer.
  • b. Pay-slip for the month before retirement (or any of the last 3 months) or evidence of remuneration.
  • c. Letter from ex-employer confirming status of outstanding remittance (Accrued Pension Right)/ Retirement benefit registration slip (public sector).
  • d. Age Declaration/Birth Certificate.
  • e. One passport photograph.
  • f. Bank Account Details (Banker’s Confirmation/Statement of Account/Copy of Cheque leaf).
  • g. Duly completed Programmed Withdrawal agreement document/ Annuity policy (to be issued by PFA/Insurance Company of Retiree’s choice upon issuance of the Benefit breakdown).
  • h. Duly Application form.

PRIVATE ABOVE AGE 50 APPLICATION

PUBLIC ABOVE AGE 50 APPLICATION

Customers with RSA balance of N550, 000 or less: Enbloc Payment:
  • a. Official notice of retirement from employer.
  • b. Letter from ex-employer confirming status of outstanding remittance (Accrued Pension Right)/ Retirement benefit registration slip (public sector).
  • c. Age Declaration/Birth Certificate.
  • d. One passport photograph.
  • e. Bank Account (Banker’s Confirmation/Statement of Account/Copy of Cheque leaf)
  • f. Duly completed Application form.

PRIVATE ABOVE AGE 50 APPLICATION

PUBLIC ABOVE AGE 50 APPLICATION

Retirement on Medical Grounds:
  • a. Official notice of retirement from employer (based on Medical Condition).
  • b. Medical Report on medical condition.
  • c. Pay-slip for the month before retirement (or any of the last 3 months) or evidence of annual remuneration.
  • d. Letter from ex-employer confirming status of outstanding remittance (Accrued Pension Right)/ Retirement benefit registration slip (public sector).
  • e. Age Declaration/Birth Certificate.
  • f. One passport photograph.
  • g. Bank Account Details (Banker’s Confirmation/Statement of Account/Copy of Cheque leaf).
  • h. Duly completed Programmed Withdrawal agreement document/ Annuity policy (to be issued by PFA/insurance Company of Retiree’s choice upon issuance of the Benefit breakdown).
  • g. Duly completed Application form.

Death Benefit/Missing Persons
  • a. Certified True of Letter of Administration or Will admitted to Probate;
  • b. Passport Picture of the deceased and Next Of Kin (NOK) (1 each);
  • c. Death Certificate or evidence of Death;
  • d. Police Report (If death is by accident, OPTIONAL);
  • e. Estate Account confirmation letter in the name of the deceased participant issued by the bank addressed to Leadway Pensure PFA LTD; OR
  • f. Administrator’s designated bank account.
  • g. Duly completed Application form.
  • h. Where not all administrators on an LOA are signatories to the Account of the deceased, Power of Attorney duly certified by the probate registry MUST be provided by those administrator(s) who wants to transfer their powers to Co-Administrator(s)
  • h. Next of Kin shall provide a satisfactory means of identification which can be any of the following;
  • i. National Identity Card or
  • j. Permanent Voters Cards
  • k. Current International Passport
  • l. Drivers’ Licence
  • m. Notary Public of Identity
  • For missing Employee’s benefit, the following are additional requirements to be submitted along with the list stated above (with the exception of Death certificate):-
  • o. Police report
  • p. Newspaper publication
  • q. Letter from ex-employer confirming that the employee has been missing and details

PRIVATE- DEATH BENEFIT APPLICATION FORM

PUBLIC- DEATH BENEFIT APPLICATION FORM

Foreigner
  • a. One passport picture
  • b. Separation Letter
  • c. Proof Enrolment with LPPFA
  • d. Letter from ex-employer on Accrued Rights of the customer and relocation
  • e. Document validating bank details
  • f. Age declaration/birth certificate
  • g. Copy of valid international passport
  • h. Duly completed Application form

Temporal/25% Lumpsum Payment
  • a. Official notice of retirement from employer
  • b. Letter from ex-employer confirming status of outstanding remittance (if any).
  • c.Age Declaration/Birth Certificate.
  • d. One passport photograph.
  • e. Bank Account (Banker’s Confirmation/Statement of Account/Copy of Cheque leaf)
  • f. Duly completed Application form

  • a. One Passport photograph
